New U.S. Coast Guard marine casualty reporting forms

The U.S. Coast Guard has posted new forms for the reporting of marine casualties.

  • CG-2692 is used for Report of Marine Casualty, Commercial Diving Casualty, or OCS-related casualty
  • CG-2692A is the Barge Addendum
  • CG-2692B is used for Report of Mandatory Chemical Testing following a Serious Marine Incident involving Vessels in Commercial Service
  • CG-2692C is the Personnel Casualty Addendum
  • CG-2692D is the Involved Persons and Witnesses Addendum
